ENTERING YOUR FACILITY INFO:
F) SET UP / Facility
a) From the Top Navigational Menu, select ‘Set Up’, then ‘Facility’.
b) Select ‘Add’ to open a blank ‘Edit New Facility Data’ form.
c) Code: Create a shortcut/nickname to reference this Facility. (Used by
the software in various screens and reports).
d) Facility: Enter the name of the facility.
e) NPI: Enter the facility’s NPI number.
f) Medicaid ID: Enter the facility’s Medicaid ID.
g) Medicare ID: Enter the facility’s Medicare ID.
h) Blue Shield ID: Enter the facility’s Blue Shield ID.
i) Place of Service: Select the Place of Service code (HCFA 1500 Claim
form Box 24b) for services rendered at this facility.
j) Enter the Address, Zip, City, State, Phone, and Fax numbers in the
fields provided.
Quick Tip! You can also add a new Facility while adding Claims by
selecting ‘Add’ in the Facility drop down menu in the ‘Edit New Claims
Data’ screen.
