  ENTERING INSURANCE INFORMATION


ADDING PATIENT'S INSURANCE DATA:


C) PATIENTS/ Insurance Tab
a) SSN: Enter the patient’s Social Security Number.
b) Sex: Select (F) Female or (M) Male.
c) D.O.B.: Enter the patient’s Date of Birth.
d) Employment Status: Select the patient’s Employment Status.
e) Marital Status: Select the patient’s Marital Status.
f) Signature on file: A check mark will default in this box; uncheck the box if there is no signature on file.
g) Provider: Select the appropriate provider for this patient. (Note: the software will automatically default to the last provider selected for the previously entered patient.).
h) Referring: Select the appropriate Referring Physician or add a new one (refer to section ‘L) SET UP/Referring Physician’).

i) Insurance: Select ‘Yes’ or ‘No’. If ‘Yes’ continue, if 'No' skip to 'Files')
j) Primary Patient Insurance: Select the ‘Edit’ button, and complete the following: a. Insurance: Select the patient’s insurance company from the drop down menu, or click ‘Add’ to add a new one (refer to section ‘II

G) SET UP/ Insurance’).
b. ID #: Enter the patient’s Insurance ID number.
c. Group #: Enter the patient’s Insurance Group ID number.
d. Employer: If this is an employer sponsored insurance plan, enter the employer’s name in this field.
e. Start: Enter the start date of the insurance coverage.
f. End: Enter the end date of the insurance coverage (if provided).
g. Co-Pay: Enter the amount or percentage as applicable.
h. Insured same as Patient: If the patient is not the insurance holder, select ‘No’ and fill out the appropriate fields.
i. OK and Go Secondary: If patient has a Secondary Insurance, click this button and follow steps a-h just taken.
j. OK: If there is no other insurance for this patient, click ‘OK’ to return to the ‘Patient Data’ screen.
